Requests for Disclosure, Correction, and Suspension of Use

At our company, we respond to requests from the individual or their representative regarding the retained personal data for "notification of intended use and disclosure," "correction, addition, or partial deletion," "suspension of use or third-party provision, and complete deletion," and "disclosure of third-party provision records" according to the following procedures.

(1) Please send a request form along with (2) identification documents (for agents, please include documents proving representation and a power of attorney). (3) If necessary, please enclose the handling fee and send it by mail. (The postage cost will be borne by the requester.)

Please note that if there are any unclear contents in the required documents, we may contact you for clarification. Furthermore, if the request is made by an agent, we may contact the individual for confirmation.

(3)Handling Fee
In the case of requests for disclosure of retained personal data, please enclose 800 yen worth of postage stamps with the application documents as a postal fee for each request. To ensure that the response reaches the individual or their representative, we will send the response via registered mail with delivery confirmation or through the prescribed electronic method to the address you provided. (No fee is required for the disclosure of electronic records or for other types of requests.)
Please note that if the required 800 yen worth of postage stamps is not enclosed or is insufficient in your request for the disclosure of retained personal data, we will inform you of this. However, if the required stamps are not provided within the specified period, we will not be able to respond to your request.

(4)Response Method
Our company will, in principle, respond in writing to the address specified by the requester within 30 days after receiving the complete set of request documents. For those who have requested a response via electronic records, we will respond according to the selected disclosure method.

(5)Cases Where Requests Cannot Be Fulfilled
There may be cases where we cannot respond to requests for the following reasons. In such cases, we will clearly state this in our response regarding the request. Additionally, please note that handling fees cannot be refunded.
① If the identity of the individual cannot be confirmed
② If the authority of the agent cannot be verified for requests made by an agent
③ If there are deficiencies in various request forms or identification documents
④ If the handling fee has not been paid
⑤ If there is a risk of harm to the life, body, assets, or other rights and interests of the individual or a third party
⑥ If there is a risk of significantly hindering the proper execution of the company's operations
⑦ If fulfilling the request would violate other laws and regulations

The personal information of the requester and their agent provided in the various request forms will be used solely for responding to those requests and for improving personal information protection at our company.
